Active Reporting of Adverse Drug Reactions

Any change in antiretroviral therapy is a clinically important event.  In order to monitor treatment success, the BC-CfE Drug Treatment Program records all reasons for changing HIV medication regimens.  Even well known or minor adverse drug reactions are important if the patient cannot tolerate therapy.  The two Drug Treatment Program forms listed below make it easy to report reasons for medication change as a routine part of clinical practice.

 

HIV/AIDS Drug Request Prescription Form

Who can report: Any physician who prescribes antiretroviral medications

What to report: Any suspected adverse drug reaction, medication intolerance, or drug interaction that results in a change to the antiretroviral regimen

How to report: Complete the "Reason(s) for medication regimen change" on the prescription form.  Prescription forms may be obtained from the Drug Treatment Program: 604-806-8515

 

Antiretroviral Therapy Discontinuation/Interruption Alert

Who can report: The Drug Treatment Program will mail an Antiretroviral Therapy Discontinuation/Interruption Alert to the patient's physician if the patient does not refill his or her antiretroviral medication for more than one month after the supply is expected to run out.  This alert provides an opportunity for enhanced patient follow-up.

Discontinuation alerts may also be generated if the reason for medication change was not specified on a prescription form.

What to report: Any suspected adverse drug reaction, medication intolerance, or drug interaction that results in a change to or interruption of antiretroviral therapy

How to report: Complete the "Reason(s) for medication discontinuation/interruption" on the form
